Comparative Analysis of the Effectiveness of Maternal and Infant Health Services in Türkiye According to Statistical Territorial Classification

This study aims to comparatively evaluate the efficiency of maternal and child health services provided across 12 regions at the NUTS-1 level in Türkiye and to identify structural differences between regions. In the analysis, regions were treated as Decision Making Units (DMUs). Input variables included the cesarean section rate, antenatal and infant follow-up rates, the number of neonatal intensive care unit beds, and the number of midwives for the year 2023. Output variables consisted of the inverse values of infant mortality rate, neonatal mortality rate, and maternal mortality ratio. An input-oriented Data Envelopment Analysis (DEA) approach was employed, utilizing both CCR and BCC models, along with scale efficiency calculations. The findings indicated that 9 out of 12 regions were technically efficient according to the BCC model, whereas the number of efficient regions decreased to 7 under the CCR model. The West Black Sea, Aegean, and Central Anatolia regions were observed to be inefficient in both models. The sources of inefficiency were observed to vary across regions in terms of technical and scale-related factors. Improvement analyses suggested that high cesarean section rates and the imbalanced distribution of midwives were among the main determinants, while neonatal mortality rate was identified as a key area requiring improvement. Overall, it was inferred that regional disparities exist in the efficiency of maternal and child health services in Türkiye, and that inefficiencies are more closely related to resource planning and management processes rather than resource scarcity.
